Key Points
- 25% Average Weight Loss: CagriSema was associated with average weight reduction of 25% after 68 weeks among approximately 2,300 women in a post-study analysis, compared with 3% for placebo.
- Consistent Results Across Menopause Stages: Reported weight loss ranged from 23.1% to 26.7% across premenopausal, perimenopausal, and postmenopausal women under the analysis's continued-treatment assumption.
- Billions in Potential Sales: FactSet consensus estimates project $11.84 billion in obesity-treatment sales for CagriSema in 2028, although investor confidence will depend on further evidence of efficacy and differentiation.
Novo Reports New Evidence for CagriSema
Novo Nordisk reported new findings on Tuesday suggesting that CagriSema, its experimental next-generation obesity injection, could deliver substantial weight loss among women with overweight or obesity.
In a post-study analysis involving approximately 2,300 women, participants receiving CagriSema lost an average of 25% of their body weight after 68 weeks, compared with 3% among those receiving a placebo. These results reflect an ideal treatment scenario in which all patients remained on the drug without interruptions.
The findings add to evidence from the company’s late-stage clinical development program as Novo seeks to strengthen its position in the competitive obesity-treatment market. However, the company noted that further work is needed to confirm the hypotheses generated by the two post-study analyses.
Weight Loss Across Menopause Stages
A second analysis examined approximately 1,900 women according to their menopausal status. At 68 weeks, average weight loss reached 26.7% among premenopausal women, 24.8% among perimenopausal women, and 23.1% among postmenopausal women, assuming continued treatment without interruptions.
The corresponding placebo groups experienced approximately 2% weight loss across the three categories.
Novo also reported reductions in waist circumference, indicating lower abdominal measurements among women overall and across menopause stages.
The findings are relevant because weight management can become more challenging during the menopausal transition. Declining estrogen levels may contribute to changes in metabolism, weight distribution, and muscle maintenance. The analyses suggest that CagriSema may support substantial weight reduction across different stages of menopause, although additional research is needed to establish the findings more firmly.
Combination Therapy Targets Two Biological Pathways
CagriSema combines semaglutide, the active ingredient in Novo’s Wegovy and Ozempic products, with cagrilintide, which targets the amylin pathway. Amylin is a hormone released by the pancreas alongside insulin and plays a role in regulating hunger and fullness.
By combining the two mechanisms, Novo aims to develop a treatment that can improve upon the weight-loss results associated with existing GLP-1 therapies.
Initial results from the phase 3 REDEFINE 1 trial showed average weight reduction of 22.7% after 68 weeks with CagriSema, compared with 2.3% for placebo. Approximately 67% of the trial’s roughly 3,400 participants were women.
The new analyses suggest that weight loss among women may be somewhat higher than the overall trial average. However, the results should be interpreted in the context of their post-study design and treatment assumptions.
Investor Expectations and Commercial Outlook
CagriSema’s development comes as Novo Nordisk and Eli Lilly expand research into amylin-based obesity treatments, seeking additional growth opportunities beyond existing GLP-1 medicines.
Despite the latest findings, investor enthusiasm for CagriSema has been restrained. Previous late-stage results fell short of some market expectations, weighing on perceptions of the drug’s potential competitive advantage.
Novo continues to argue that the treatment offers sufficient efficacy and differentiation to attract significant demand. Analysts expect CagriSema to generate $11.84 billion in obesity-specific sales in 2028, according to FactSet consensus estimates.
The commercial outlook will depend on the treatment’s final clinical profile, regulatory progress, competitive positioning, and ability to establish demand in a market where expectations for weight-loss efficacy remain high.
What Investors Should Watch Next
Further confirmation of the post-study findings will be important for evaluating CagriSema’s potential, particularly its performance across menopause stages and its effects on abdominal fat. Investors will also monitor clinical development milestones, regulatory progress, and evidence that the combination can differentiate itself from existing obesity treatments.
The results strengthen the evidence supporting CagriSema’s potential, but they do not resolve all questions surrounding its competitive position. Its long-term commercial value will depend on whether the clinical benefits translate into a compelling treatment profile and sustained demand.
