Medicare Coverage Expands the GLP-1 Market

Eli Lilly is seeing significant early demand from Medicare beneficiaries following the launch of expanded obesity-drug coverage in July. CEO Dave Ricks said approximately 700,000 new seniors have started GLP-1 treatments since the rollout began, suggesting that insurance access is bringing a substantial number of new patients into the market.

The coverage operates through a temporary program known as “Bridge,” allowing eligible beneficiaries to obtain obesity medicines for a $50 monthly co-pay. Medicare serves approximately 66 million people, including individuals aged 65 and older and people with disabilities, creating a potentially large market for weight-loss treatments.

Lilly Captures Most New Patients

According to Ricks, around 70% of the seniors who have newly started GLP-1 treatments through the program are using Lilly medicines. The figure provides an early indication of how the company’s products are performing against those offered by its main competitor, Novo.

Lilly’s Zepbound appears to be a significant beneficiary of the expanded coverage. Eligible seniors can access the injectable alongside Lilly’s newly launched Foundayo pill, while Novo offers its competing Wegovy injection and pill through the same program.

Zepbound Targets Patients With Greater Medical Needs

Ricks said many physicians continue to focus on patients with higher levels of body weight and more obesity-related complications, an area where Lilly sees Zepbound playing an important role.

The positioning could give Zepbound a strong role as Medicare coverage expands. Rather than simply increasing the number of prescriptions, broader coverage may also change which patients are able to begin treatment based on medical need and physician priorities.

Foundayo Adds an Oral Option

Lilly’s newer Foundayo pill is targeting a somewhat different part of the obesity-treatment market. Ricks said the oral medicine is particularly relevant for patients seeking a more convenient treatment option and those looking to lose approximately 25 to 30 pounds.

Foundayo launched in April, several months after Novo’s competing oral treatment. Despite entering the market later, Ricks said the Lilly pill currently accounts for one-third of new patients taking oral GLP-1 medicines.

Coverage Rollout Appears to Be Gaining Traction

The initial rollout has also provided an important test of how effectively the Medicare system can expand access to obesity medicines. Ricks said he has not heard of significant logistical problems, crediting the Centers for Medicare and Medicaid Services with educating physicians and coordinating with drugmakers and the insurance system.

For Lilly, the early enrollment figures could become increasingly important as Medicare coverage expands the pool of potential patients. The company’s ability to retain its share of new prescriptions while developing additional oral and injectable treatments will shape its position in the rapidly growing GLP-1 market.

What Investors May Watch Next

The next phase will depend on whether the initial 700,000-patient expansion continues and whether Lilly can maintain its reported 70% share of new Medicare patients. Investors may also monitor the relative adoption of Zepbound and Foundayo versus Novo’s products, prescription trends among seniors and how the temporary coverage program evolves. The scale of Medicare access could make coverage policy an increasingly important factor in the competitive outlook for obesity medicines.
